Swing Bed Rehabilitation


Sheridan Community Hospital is dedicated to providing individual-based professional health care for the families in our community. The Swing Bed Rehabilitation Program allows small, rural hospitals, like Sheridan Community Hospital, provide skilled nursing and rehabilitation services within our community. We provide specialized care to help patients recover from illness, wounds, stroke, injury, or surgery in a hospital setting versus a nursing home.

Our in-house hospitalists and ER physicians are onsite 24/7 to assist Registered Nurses with treating patients, ordering onsite labs and imaging studies, and beginning intravenous antibiotics treatment. Even if you were not originally hospitalized at Sheridan Community Hospital, you may still qualify for our Swing Bed Rehabilitation Program.

WHAT ARE QUALIFICATIONS FOR THE SWING BED REHABILITATION PROGRAM?

The Swing Bed Rehabilitation Program is federally approved.

To be eligible, patients must:

  • Have had a qualifying three midnight inpatient hospital stay within the last 30 days (Dependent on insurance)
  • Have a qualifying insurance plan
  • Need daily rehabilitation needs, which could include nursing care and physical and occupational therapy

Swing Bed Rehabilitation

Physical Therapy
Our team of physical therapists work to rehabilitate your injuries, build strength, prevent falls, and assist in your recovery. They will help you resume regular life activities such as walking, exercising, building strength, increasing flexibility, and maintaining your balance.

Occupational Therapy
Occupational therapists will work with you to restore daily living skills such as bathing, dressing, grooming, cooking and basic housekeeping so you can take care of yourself again.

Speech Therapy
Our speech therapists will help you recover your communication skills such as expressing your thoughts orally, reading, and safe swallowing practices.

Nutrition Services

Nutrition Services
Our registered dietitian will work with you to develop a personalized nutrition plan. We serve fresh food that tastes great and our customized plan will meet your dietary needs and personal preferences.

Patient Care Coordination and Social Work
Our Patient Care Coordinator and Social Worker will help you and your family connect with the resources and services you may need during your stay through discharge.

 

WHAT IS THE COST FOR THE SWING BED REHABILITATION PROGRAM?

Most Medicare plans have a $0 deductible for the first 20 days; therefore, there is no out of pocket expenses for the first 20 days. Days 21-100 will require co-insurance rates, which depend on your health care coverage.
